Step 1 – Kindly visit our website www.icmcne.com

Step 2- Click the ‘’MEDICAL RECORDS TAB’’

Step 3- Fill out the medical records request

Step 4- After filling out your data and sending your request download ‘’CLAIM SLIP’’

Step 5- You will receive an SMS (text message) from Health Information Management Department regarding the date of availability of your requested Health Record and the requirements needed upon release.
